Spoilers ahead. Silent Hill: Townfall is set in 1996 in St. Amelia, a Scottish island town revived by the oil company CEG. A fire on its SWDP rig killed every worker except Simon Ordell, the outside safety inspector. Nurse Zoe Ellis kept him alive, and CEG's CEO, Richard Shaw, who had ignored a faulty part to avoid downtime, paid for Simon's heart transplant. The price was false testimony blaming a dead engineer. The fog-bound St. Amelia you play through is a purgatory where Simon, Zoe and Richard relive their guilt. The ending shows whether Simon accepts his part in the cover-up.

Before the game: CEG and the SWDP disaster
St. Amelia was a fading fishing town until Chapman Energy Group (CEG) arrived. It built a refinery, pipelines and offices to serve its SWDP oil rig, gave the town thousands of jobs and funded a ward at the medical centre. The town owed the company everything, which is why nobody questioned it.
Richard Shaw ran CEG by cutting costs. Game Rant says a faulty part on the rig, an RCU, would have cost millions and weeks of downtime to fix, so Richard had "his usual guy" do the safety visits and the fault was overlooked. After a whistleblower raised concerns, Simon Ordell was brought in from the US as an independent inspector.
Shortly after Simon arrived, the rig caught fire. Everyone on it died except Simon. The spill and the energy crisis that followed cut the island off, and the small medical centre was overwhelmed.
Simon, Zoe and Richard: who they really are
Every character and voice actor
Simon Ordell
The sole survivor, with a damaged heart and a survivor's guilt built around the number of dead. He was there to protect the workers and failed. To live, he took Richard's offer of a transplant, and the debt came with a condition: tell the tribunal that the dead engineer Joe McKenzie was at fault. He agreed, then began to doubt it. That doubt is the whole game.
Zoe Ellis
An inexperienced nurse who was left to run a ward through the disaster. She performed surgery on Simon she was not trained for and kept him alive for more than five days until he was airlifted to the mainland. Other patients died around her, she blames herself for a dosing mistake, and she wants to resign and leave the island but cannot.
Richard Shaw
CEG's CEO, suspended by the board after the disaster. He sees himself as the man who saved St. Amelia, not the one who ruined it. Buying Simon a heart was, in his eyes, an investment. When Simon wavers, Richard threatens him with a scalpel. His personal hell is his own office tower, which is why Part 5 is spent inside CEG Tower.
Dr. Glenn and the Patient
VGTimes reads Dr. Glenn as the doctor who insisted Simon be saved even at the cost of other patients, leaving Zoe no choice. The Patient Zoe tends in Part 3 is a distorted version of Mr. Stewart, an elderly patient she could not save, whose death VGTimes ties to Simon's donor heart.
What the fog town is
The St. Amelia you explore is not the real town. Wikipedia describes it as a purgatory-like version of the town in which Simon, Zoe and Richard are trapped, each reliving what they are guilty of: Simon his part in the cover-up, Zoe the patients she could not save, Richard his refusal to accept any blame.
That is why the game hands control between them. Zoe's chapters (5 and 7) replay her night on the ward, Part 5 traps Richard in his own tower, and the rest belongs to Simon. Zoe spells out the rule to Simon: this place lets him accept the past, not change it. The Chapter 15 name, Trying Again, and the trophy Can't Change Things say the same thing.
What the monsters mean
Interpretation, following VGTimes. The game does not label any of this.
- The Weight: Simon's own guilt, bent under a load. You pull three metal rods out of it in the final fight; VGTimes reads them as survivor's guilt, the deal with Richard and the betrayal of his job
- The Widows: their bodies carry photographs of the dead, so they stand for the families who lost people on the rig. They hunt Richard, not Simon
- The Patient: the burned figure with an IV line, Mr. Stewart merged with the fire's victims
- Falling and crawling bodies: the workers who jumped from the burning rig
- The giant legs in the fog: the company itself, looming over the town
- The Collapse: the rig-like giant that appears at the end, which Game Rant calls the embodiment of St. Amelia's rage
What each ending means
All three main endings play out on the pier with the Flare Gun. Simon can shoot Richard, or fire into the sky and leave Richard to the town. Which one happens is decided by your hidden score, not by a button press.
How the hidden score picks your ending
Sources differ on parts of the real-world epilogues. The version above follows Wikipedia and Game Rant; Prima Games describes Richard using the scalpel in Justice and Simon using it in Abandoned.
Justice
Simon fires the flare into the sky and accepts his own share of the blame. The Collapse comes for Richard, and Zoe's locked door in Room 204 finally opens, so she can leave. In the real world, Simon refuses to lie, and Richard stabs him in his hospital bed. Richard loses his star witness and has to face the tribunal alone. Simon dies, but the town gets the truth.
Forever
Simon fires into the sky, but cannot fully accept his role. He collapses clutching his heart and wakes on the operating table with Zoe, back at the start of the loop. Zoe promises not to leave him. Dialogue suggests this has happened many times before.
Abandoned
Simon shoots Richard. Revenge fixes nothing: Richard's voice mocks him over the CRTV, and the loop does not end. Wikipedia and Prima Games describe the real-world version as Simon killing Richard in his hospital bed with a scalpel. With the man who should answer for the disaster dead, CEG escapes the tribunal.
Dog and Sci-fi (joke endings)
Both need New Game Plus and neither is canon. In Dog, a guide-dog charity statue grabs the Flare Gun and takes Simon and Richard to the pub to talk it out. In Sci-fi, Simon wakes in a cryo pod next to Richard's, and Zoe radios that she and the town are still aboard the Collapse, a ship falling into the sun.

None, officially. Series producer Motoi Okamoto said the team does not designate any single ending as canon.
The story does have a direction, though. Justice is the only main ending where the loop breaks for both Simon and Zoe, and the one the game's scoring rewards for reading documents instead of killing. Forever and Abandoned are the two ways Simon can fail to let go.
