Silent Hill Townfall Walkthrough
Updated 2026-09-20 · Category: Guide
The Silent Hill: Townfall main story runs through nine confirmed beats: escaping the Otherworld back to the town square, Zoe at the Shorefront Family Clinic, reporting to Dr. Glenn at the Medical Centre, the mission "The Patient", the transfusion in the Alternate Medical Centre, opening the road to CEG Tower, returning to the tenth-floor office, exhausting your options in the Alternate Facility, and finally defeating The Weight. Expect 10-14 hours, and expect to tune, hide and solve far more often than you fight.

Konami has not published a chapter list, but the story trophies leak the spine of the campaign in order, and two independent trophy databases record them identically. That gives a reliable skeleton to follow even before launch.
Every beat below is a checkpoint you cannot skip, because each one is a story trophy. Use it as your position marker: if you have just taken the transfusion, you are a little past the middle of the game.
| # | Story beat | What it asks of you |
|---|---|---|
| 1 | Welcome To St. Amelia | Escape the Otherworld and return to the town square |
| 2 | Resignation | Reach Zoe at the Shorefront Family Clinic and tell her you understand |
| 3 | A Long Night Ahead | Report to Dr. Glenn at the Medical Centre |
| 4 | The Patient | Complete the mission "The Patient" |
| 5 | Many Into One | Accept the transfusion in the Alternate Medical Centre |
| 6 | Key To The Castle | Open the route to CEG Tower |
| 7 | Find Yourself | Return to the office on the tenth floor |
| 8 | Can't Change Things | Exhaust every option in the Alternate Facility |
| 9 | Eraser | Defeat The Weight |
How the game opens
Simon Ordell arrives on the fog-bound Scottish island town of St. Amelia after a mysterious letter. The prologue puts the handheld CRTV in your hands in a bedroom, and the game proper starts from there.
That device is the engine of the whole walkthrough. Tuning it reveals enemies through walls and fog, and it feeds you hints about how to solve the puzzles in front of you.
The loop every chapter repeats
Townfall is not a push-forward combat game. The rhythm reported from hands-on sessions is explore, tune for a lead, slip past or remove the threat, then solve something to open the road.
Hiding is part of traversal, not a panic button. Closets, sheds and half-open doors work as cover, and you can lean out to check a patrol before committing.
Expect this loop to be slow early. One journalist spent three hours and completed a single objective: fetching a prepaid power card from a corner shop to restore power.
Combat during the route
Improvised melee weapons break. A plank lasts a handful of encounters and then it is gone, so treat every swing as spending a resource.
A revolver arrives later in the game. It hits hard, but the PlayStation.Blog report is blunt that firing it "always makes things worse", because the noise pulls more enemies toward you.
On normal difficulty, two or three hits kill you, and enemies have grab attacks that punish hesitation. Walking away is usually the correct play.

These are the only location names currently traceable to a verifiable source. The structure they suggest is a single-track route that alternates between the real town and its Otherworld counterparts.
- St. Amelia town square — the hub you return to after the Otherworld prologue
- Shorefront Family Clinic — Zoe
- Medical Centre — Dr. Glenn
- Alternate Medical Centre — the transfusion
- CEG Tower, including the tenth-floor office
- Alternate Facility — immediately before the fight with The Weight
Platform notes worth knowing before you start
On PS5 Pro the game supports PSSR and ray tracing. The DualSense uses its gyro for both CRTV tuning and aiming, which is the intended way to hunt for a signal.
Save points are phone booths, and autosaves announce themselves with the red square the series has used for years.
What this walkthrough does not claim yet
As of September 20, 2026 — four days before launch — there is no cross-verifiable step-by-step chapter walkthrough in existence. This page gives the confirmed story spine and the mechanics that carry it. Room-by-room routing, item pickups and puzzle answers land here after the September 22 early-access window, tested on our own saves. We will not publish coordinates or door codes we have not seen ourselves.
How to handle an area you have not seen before
The safe order is always the same: tune first, then look, then move. A CRTV sweep on arrival tells you where the patrols are before you have committed to a door, and it costs you nothing but a few seconds of standing still in cover.
Once you know the layout, pick your exit before you pick your objective. Closets, sheds and half-open doors are the cover this game gives you, and knowing which one you are running to matters more than knowing what you are running from.
Only then search. Documents carry puzzle codes, so a room you skipped can become a wall you cannot pass two rooms later.
Where the route gets harder
The transition into the Alternate Medical Centre is the point where the game stops being a town exploration and becomes something else. The transfusion beat, Many Into One, sits at that hinge.
From CEG Tower onward you are in the back half, and the Alternate Facility beat immediately precedes the only confirmed boss in the game.
Resource pressure compounds across this stretch. Melee weapons you broke early do not come back, and ammunition scarcity means the revolver you picked up is not a solution to the last act.
Playing for an ending while you walk the route
Townfall has five endings, and none of them is selected by a menu. The director has been explicit that the outcome tracks how you played: your aggression, your thoroughness, and the time you spent in each area.
That means your walkthrough behaviour is your ending choice. If you want a specific tone of ending, commit to a consistent style across the whole run rather than changing approach near the finale.
Three endings are reachable on a first run; the remaining two require New Game Plus, so there is no reason to agonise over it on your first pass.
Silent Hill Townfall FAQ
Is there a full chapter-by-chapter Townfall walkthrough yet?
- Not one that anyone can verify. The nine story beats above are confirmed and ordered, but room-by-room routing does not exist publicly before launch.
Do I have to fight my way through the story?
- No. The intended approach is avoidance. Melee weapons break, ammunition is scarce, and firing a gun draws more enemies toward you.
Where do I save during the walkthrough?
- At phone booths. Autosaves also fire on their own and are flagged with the series' red square.
How long is the main story?
- About 10 to 14 hours, depending on how thoroughly you explore and how often you choose to fight.
